Accident/Injury

In the event of my child suffering an injury during a club activity, I understand that the coach, team manager or supervising adult on duty will do their best to administer first aid and/or call emergency services if deemed appropriate.

By registering my child/children with Castlebar Town FC, I agree that I will:

Contact our child welfare officer Anthony McDonnell (+353 87 654 7902), before attending any club activities if I need to discuss this further. I will await a reply from the child welfare officer before attending any club activities. Otherwise it will be assumed that consent is given.

Medical Disclosure
The club does not ask for or store personal medical information. In the case of a serious medical condition, the onus is on you the parent/guardian of any underage player to ensure that coaches / supervising adults on duty on any given day is fully aware and instructed on necessary steps if you deem it appropriate. Please bear in mind that there my be different coaches / supervising adults on duty at various club activities.

Photo/Video Consent
We would like to inform you about our club's policy regarding the use of photos and videos.

Occasionally appearing in photos/videos comes hand in hand with being a member of the club. At times the club may wish to take photos or videos of teams or individuals. We adhere to child welfare guidelines to ensure these are safe and respectful and used solely for the purposes for which they are intended, which is for promotions and celebration of the activities of the club and for training. Photos and/or videos may appear on social media, the club website and in print media or promotional material.
